Using your receipt, select the date you visited BevMo!, fill in the 15-digit survey code and enter your unique 11-digit club member number.
Rate your overall satisfaction with your shopping experience at BevMo!
Answer questions and rate statements about the staff, alcohol for sale, prices, store layout and more. Continue through the survey until you are asked for your personal details.
Provide your contact details, such as your name and email and you could win one of five $50 BevMo! vouchers, drawn each month.
