It’s easy to enter the Mellow Mushroom survey. With your receipt in hand, go to https://iammellow.com and enter the 16-digit code printed on the bottom of your receipt. Now you will be able to answer questions about your last visit to a Mellow Mushroom restaurant. Questions will be asked about the customer service, restaurant atmosphere and the quality of your meal. You will also be asked a couple of questions about the Mellow Mushroom website and social media pages. Lastly, ensure you enter your personal details such as age and gender. Once complete, you will be presented with a validation code – record this on your receipt and bring it with you on your next visit to receive a special offer!
